| Sumario: | ABS TRACT Ob jec ti ve: Chro nic re nal fa i lu re is a chro nic mic ro inf lam ma tory pro cess of cyto ki ne dysre gu la ti on and in cre a sed in su lin re sis tan ce is a known con cept in the se pa ti ents. Tumor necrosis factor (TNF)-? is a pro inf lam ma tory cyto ki ne and has be en shown to in cre a se in chro nic re nal fa i lu re pa ti ents in so me stu di es. We plan ned this study to in ves ti ga te TNF-? le vels, in su lin re sis - tan ce va lu es and in ter re la ti ons in chro nic re nal fa i lu re pa ti ents. Ma te ri al and Met hods: A to tal of 60 sub jects of whom 30 had chro nic re nal fa i lu re (15 fe ma les-gro up 1) and 30 we re he althy controls (16 fe ma les-gro up 2) we re inc lu ded in the study. Se rum cre a ti ni ne, glo me ru lar fil tra ti on ra te le vels, TNF-?, fas ting plas ma glu co se, fas ting in su lin le vels we re stu di ed and HO MA-IR was cal cula ted. Re sults: Me an va lu es of glo me ru lar fil tra ti on ra te we re de tec ted as follows; gro up 1: 42.05 ± 24.60 ml/min and gro up 2: 106.67 ± 17.10 ml/min, TNF-? va lu es we re fo und as gro up1:57.65 ± 42.2 pg/ml and gro up 2:18.25 ± 7.90 pg/ml and HO MA-IR va lu es we re fo und as gro up 1:3.71 ± 2.56 and gro up 2: 2.37 ± 1.45. When HO MA-IR and TNF-? va lu es of gro up 1 and 2 we re com pa red res pec - ti vely, a sta tis ti cally sig ni fi cant dif fe ren ce was de tec ted (p< 0.01) (p< 0.001). Ho we ver a ne ga ti ve or po si ti ve re la ti ons hip was not de tec ted in bi va ri a te cor re la ti on of HO MA-IR and TNF-? (r= 0.091, p= 0.634). TNF-? was found to be an in de pen dent va ri ab le for chro nic kid ney di se a se in reg res si - on analy sis (p= 0.010). Conc lu si on: In this study, in su lin re sis tan ce was pre sent in sub jects with chro nic re nal fa i lu re and le vel of TNF-? in cir cu la ti on was hig her com pa red to con trols wit ho ut chro nic re nal fa i lu re. Ho we ver, TNF-? did not have an influence on in su lin re sis tan ce.
