Summary:The methodology and approach to conceptualization, measurement, and analysis of outcomes questions in health care is complex. The movement towards measurement of outcomes in health care has been precipitated by the changing market and reimbursement conditions, an enhanced sense of accountability, and the need to improve the knowledge base in medicine. However, the process of outcomes management has remained an enigma for many individuals and contains several methodological challenges. Study designs, outcomes approaches, and risk adjustment are reviewed. An overview of essential first steps to follow in approaching the outcomes question are discussed.
