Even when dermatologic consultation is indicated, the primary care physician plays a key role in identifying underlying associated diseases and managing complications of systemic therapy. Pyoderma gangrenosum (PG), for example, most often manifests as one or more ulcers on the legs of persons with such underlying conditions as inflammatory bowel disease, arthritis, or lymphoproliferative disease. PG is often initially misdiagnosed as a venous or arterial ulcer; a biopsy distinguishes the condition from clinical mimics.
