HIV prevalence was 15.4 % (95 % CI 11.6-20.1), with only 45.8 % previously aware of their infection. HIV prevalence achieved 6.4 % among youth 18-24 years and was higher among MSM with lower socio-economic status. In multivariate analysis, correlates of HIV were older age, gay identity, lower socio-economic status, social networks with HIV-positive MSM, receptive anal sex, and internet sex partners. Policy change towards increasing avenues for HIV testing coupled with antiretroviral treatment regardless of CD4 count or stage of disease stand to benefit the MSM community if scaled up fast enough Hemos conducido una encuesta de comportamiento con vinculacion serologico, de muestreo por tiempo-lugar, con hombres que tienen sexo con hombres (HSH) en São Paulo, Brasil, la ciudad más grande de América Latina y del Hemisferio sur (N = 1,217 entrevistados y resultados serológicos para 771). La prevalencia de VIH fue 15.4 % (IC 95 % 11.6-20.1), 45.8 % previamente conocian su infección. La prevalencia de VIH alcanzó 6.4 % entre jóvenes de 18 a 24 años y fue mayor entre los HSH de nivel socioeconómico más bajo. En el análisis multivariado, se encontró correlación entre VIH y mayor edad, identidad sexual gay, bajo estado socio-económico, redes sociales con un HSH con infección por VIH, sexo anal receptivo, y parejas sexuales conocidas por internet.
