| Sumario: | Image-guided adaptive radiation therapy (IGART) is an evolving approach to cancer treatment and represents the latest effort to fulfill a longstanding goal for radiation oncology: escalated therapeutic doses of ionizing radiation that are delivered precisely in time and anatomic space to tumor tissue while sparing healthy, nontarget tissues to maximize the therapeutic ratio. This article introduces and reviews the concepts, history, radiobiology, targeting, technological, planning, delivery, and plan-adaptation principles and approaches to IGART, with a focus on the increasingly important roles of functional imaging modalities in the IGART process.
