      ab: PURPOSE: To report patient acceptance of prognostic fine needle aspiration biopsy (FNAB) for uveal melanoma. METHODS: Retrospective analysis of data derived from 200 consecutive patients with uveal melanoma (11/2013 to 7/2016). Patients underwent prognostic FNAB in addition to routine standard-of-care treatment. Prognostication was performed using commercially available Multiplex Ligation-dependent Probe Amplification (MLPA, Impact Genetics; Toronto, Canada) or Gene Expression Profiling (GEP, Castle Biosciences; Dallas, TX). RESULTS: Tumors were located in the iris (15, 7.5%), ciliary body (30, 15%), or choroid (155, 72.5%) (see Table 1). The majority of patients underwent plaque radiation therapy (147, 73.5%) or enucleation (41, 20.5%). The remaining were treated by iridectomy (9, 4.5%), transpupillary thermotherapy (2, 1%), or proton beam radiation (1, 0.5%). FNAB was offered to 145 patients (72.5%). Seven patients (5%) refused prognostic FNAB. Of the 138 patients who underwent FNAB, 76 were classified as being at low risk for metastases; 57 (41%), at high risk; and in 5 patients (4%) there was technical failure to process the sample. CONCLUSIONS: The vast majority of patients (95%) undergoing treatment for uveal melanoma accept to undergo prognostic FNAB when offered. The overall technical failure rate is low (4%) but should be mentioned during patient counseling. Only about 30% of cases (those at high risk for metastases) would be eligible to enter any available adjuvant therapy trial.
