| Sumario: | Problem: Horizon Health Network, the largest Health Authority in the Province of New Brunswick, is composed of four large areas, each of which includes several health care facilities. In 2014, each area had multiple CVAD and PFV policies, and a variety of educational materials. Many of these policies and educational materials were very outdated. The Clinical Nurse Specialist for Family Medicine and a Nursing Clinical Educator were taxed with creating standardized nursing policies to address adult and pediatric populations across the entire network. In addition, educational plans were developed to address the learning needs of nurses caring for these patients/clients. Approach: Working with our Nursing Practice Consultants, we brought together a group of clinical experts from the four areas, representing various specialities. Current best practice guidelines and experts from across Canada were consulted. The working group met monthly through videoconferencing in order to create evidence-based policies and innovative learning packages. Results: In the summer of 2015, comprehensive nursing policies were approved by stakeholders. A PIV learning module was made available via the Horizon Health Network Intranet site. The CVAD certification program, including four interactive e-learning modules and standardized handson learning, was created and made available to nurses across the network. In the fall of 2016, a train-the-trainer “road show” officially launched implementation of the CVAD policy and learning modules. C onclusion: T hrough collaboration and teamwork, Horizon Health Network overcame the challenges of being a geographically large Health Authority and successfully standardized care, fulfilling Horizon Health Network’s Vision: Exceptional care.
