Sumario:This case study outlines the management of the peristomal skin complications encountered in an obese patient who developed a highoutput ileostomy, following an anterior resection for perforated diverticular disease. Factors such as large skin creases, a retracted stoma, lumen below skin surface, dehydration, electrolyte disturbances and time-consuming appliance changes made the care more difficult, requiring innovative treatment planning. Peristomal skin trauma from erosion and faecal moisture-associated dermatitis, by definition is a wound, so implementing topical negative pressure wound therapy (TNPWT) could ideally promote its healing and improved patient outcomes. This case study describes how applying TNPWT, in this case KCI vacuum-assisted closure (VAC®), to complex peristomal skin trauma not only initiated the healing process but also improved the quality of life of a woman exhausted by her clinical conditions.
