        Objective: To evaluate the changes in blood pressure and heart rate induced by orthostasis in individuals with prediabetes. Methods: We studied 27 patients with prediabetes (age: 47.52 ± 9.87 years) and 31 with normal glycaemia (age 46.39 ± 9.00 years), assisted at the National Institute of Endocrinology. Electrophysiological records were completed at the Institute of Neurology and Neurosurgery. Oscillometric blood pressure and heart rate of Colin Pilot polygraphic record system were read in supine position and at the first, third and fifth minutes after active orthostasis. The values registered in both groups were compared by means of variance unifactorial analysis and the test of differences between proportions. Results: Subjects with prediabetes showed higher systolic blood pressure values than normoglycemic controls in the supine position (124 ± 13 vs. 115 ± 12 mmHg, p= 0.010) in the first minute of orthostasis (129 ± 19 vs. 117 ± 15 mm Hg; p= 0.008). The showed lower percentage of increase in heart rate than controls in the first minute (8.0% vs. 16.5%; p= 0.003) and third minute (8.55) vs. 18.36%, p= 0.007) of orthostasis. There were no significant differences between groups in relation to other parameters of blood pressure and heart rate. Conclusions: Individuals with prediabetes were able to adequately regulate blood pressure during orthostasis, which suggests normality of peripheral sympathetic vascular function. The alterations observed in the control of heart rate during orthostasis are not conclusive to demonstrate cardio vagal injury, as they could be influenced by higher supine baseline blood pressure recorded in subjects with prediabetes.
        Objetivo: Evaluar los cambios de tensión arterial y frecuencia cardiaca inducidos por la ortostasia en individuos con prediabetes. Métodos: Se estudiaron 27 pacientes con prediabetes (edad: 47,52 ± 9,87 años) y 31 con glucemia normal (edad 46,39 ± 9,00 años), atendidos en el Instituto Nacional de Endocrinología. Los registros electrofisiológicos se realizaron en el Instituto de Neurología y Neurocirugía. Se tomó la tensión arterial oscilométrica y la frecuencia cardiaca de un sistema de registro poligráfico Colin Pilot, durante la posición supina y al primer, tercer y quinto minutos después de pasar a la ortostasia activa. Los valores registrados en ambos grupos se compararon mediante un análisis de varianza unifactorial y el test de diferencias entre proporciones. Resultados: Los sujetos con prediabetes mostraron valores de tensión arterial sistólica mayores que los controles normoglicémicos en la posición supina (124 ± 13 vs. 115 ± 12 mmHg; p= 0,010) y en el primer minuto de la ortostasia (129 ± 19 vs. 117 ± 15 mm Hg; p= 0,008), y un menor porcentaje de incremento de la frecuencia cardiaca que los controles en el primer minuto (8,0 vs. 16,5%; p= 0,003) y tercer minuto (8,55 vs. 18,36%; p= 0,007) de la ortostasia. No se registraron diferencias significativas entre grupos en relación con los otros parámetros de la tensión arterial y la frecuencia cardiaca. Conclusiones: Los individuos con prediabetes fueron capaces de regular adecuadamente la tensión arterial durante la ortostasia, lo cual sugiere normalidad de su función simpática vascular periférica. Las alteraciones observadas en el control de la frecuencia cardiaca durante la ortostasia no son concluyentes para demostrar la lesión cardiovagal, pues pudieran estar influenciadas por la mayor tensión arterial basal supina registrada en los sujetos con prediabetes.
