        Purpose: The diagnostic accuracy of small cell lung carcinoma (SCLC) using combinations of Thinprep® bronchial wash and brush cytology, plus or minus a transbronchial lung biopsy (TBLB) is unknown. Study Design and Methods: Wash, brush and biopsy specimens from subjects with SCLC and without lung cancer (Negative) were identified by linking laboratory and Cancer Registry files. Subjects were subdivided based on whether they had 1 diagnostic wash or brush (A) or both specimens (B). Worst diagnoses based on: #1) 1 wash or brush, #2) 1 wash and brush, #3) 1 wash or brush plus TBLB and #4) 1 wash and brush plus TBLB were compared to the Registry status. Sensitivity, specificity, positive (PPV) and negative (NPV) predictive value estimates were calculated. Results: Group A SCLC had 24 subjects and Group B 99. Group A Negative numbered 577 and Group B 238. Sensitivity was highest when cytology was combined with TBLB and highest at 80% with a combination of 1 concurrent wash, brush and TBLB. This combination also had the best overall Specificity, PPV and NPV (>92%). Conclusions: One Thinprep® wash and concurrent brush sample plus a TBLB are an optimal combination to diagnose SCLC and exclude those without lung cancer.
        Objet: L'exactitude des diagnostics de carcinome pulmonaire à petites cellules établis à l'aide de combinaisons de cytologies pratiquées sur des échantillons de lavage et de brossage bronchiques traités au moyen du système Thinprep®, accompagnées ou non d'une biopsie transbronchique, est inconnue. Plan et méthodes de l'étude: Des échantillons prélevés par lavage, brossage et biopsie sur des sujets atteints d'un carcinome pulmonaire à petites cellules et des sujets sans cancer du poumon, c'est à dire des sujets négatifs, ont été sélectionnés en liant des dossiers de laboratoire et des dossiers du Registre du cancer. Ces sujets ont été subdivisés selon que l'on disposait d'un lavage ou d'un brossage diagnostique (group A) ou des deux prélèvements (groupe B). Les pires diagnostics - basés sur 1) un lavage ou un brossage, 2) un lavage et un brossage, 3) un lavage ou un brossage et une biopsie transbronchique et 4) un lavage, un brossage et une biopsie transbronchique - ont été comparés aux cas du Registre. Des estimations de la sensibilité, de la spécificité et des valeurs prédictives positives et négatives ont été calculées. Résultats: Le groupe A comptait 24 sujets porteurs d'un carcinome pulmonaire à petites cellules, et le groupe B, 99. Le groupe A comptait 577 sujets négatifs, et le groupe B, 238. La sensibilité était plus élevée lorsque la cytologie était combinée à une biopsie transbronchique, et encore plus élevée, atteignant 80 %, lorsqu'un lavage, un brossage et une BTB étaient combinés. Cette combinaison était également associée aux meilleures spécificité et valeurs prédictives globales (> 92 %). Conclusion: Un échantillon de lavage et de brossage concomitant préparés avec la technique Thinprep® et une biopsie transbronchique constituent la combinaison optimale pour diagnostiquer le carcinome pulmonaire à petites cellules et écarter les sujets qui n'ont pas de cancer du poumon.
