        This study investigated the variations of rectal motion and wall thickness in prostate intensity-modulated radiotherapy using a novel 2D probability density function. To evaluate the impact of the position, thickness, and deformation of the rectum on the dose distribution in prostate intensity-modulated radiotherapy plans, probability density functions (pdf) of the deformation of rectal cross-section (D W), rectal dose distribution (R M), and changes in rectal wall thickness (T W) were used in the planning optimization. The problem of approximating the product of a number of Gaussian mixture distributions arises in the number of parameters describing the specific mean value, standard deviation, and weight in every Gaussian. In this work, a pdf model has been developed which specifies the variability of the average rectal wall thickness. The model is based on the histogram of 587 randomly selected patients with prostate cancer. The average wall thicknesses were determined based on the rectal structure contours drawn on the planning CT image set of the patient. The pdf describing the variability of the rectal wall thickness (pdf TW) is represented as a three-mode Gaussian mixture of specific (μ F ,σ F); (μ PF ,σ PF) and (μ E ,σ E) and individual weights (w F , w PF , and w E) representing full, partially full, and empty rectal states, respectively. A 2D Gaussian pdf of rectal motion and rectal thickness (2D pdf M&TW) function, as a product-mixture model of the pdf of the rectal motion (pdf M) and the pdf TW , was developed using published and experimental data, respectively, and presented mathematically. Using correlation values between the functions pdf M and pdf TW , the sensitivity of profiles and projections to the 2D pdf M&TW is numerically demonstrated.
        Cette étude examine les variations de mouvement rectal et d'épaisseur de la paroi rectale en radiothérapie avec modulation d'intensité (IMRT) de la prostate, en utilisant une nouvelle fonction de densité de probabilité 2D. Pour évaluer l'impact de la position, de l'épaisseur et de la déformation du rectum sur la distribution de dose dans les plans de traitement de la prostate avec IMRT plans, les fonctions de densité de probabilité (pdf) de déformation de la section transversale du rectum (D W), de distribution de dose rectale (R M) et de changement dans l'épaisseur de la paroi rectale (T W) ont été utilisées dans l'optimisation de la planification. Le problème d'établir une approximation du produit d'un certain nombre de distributions de mélanges de gaussiennes découle du nombre de paramètres décrivant la valeur moyenne spécifique, l'écart-type et le poids de chaque gaussienne. Dans cette étude, un modèle pdf a été produit pour spécifier la variabilité de l'épaisseur moyenne de la paroi rectale. Ce modèle est basé sur l'histogramme de 587 patients atteints d'un cancer de la prostate choisis au hasard. Les épaisseurs de paroi moyennes ont été déterminées à partir des contours de la structure rectale dessinée sur l'ensemble d'images de planification TDM du patient. La pdf décrivant la variabilité de l'épaisseur de la paroi rectale (pdf TW) est représenté sous la forme d'une mélange de gaussiennes à trois modes de pondérations spécifiques (μ F ,σ F); (μ PF ,σ PF) et (μ E ,σ E) et individuelles (w F , w PF and w E) représentant respectivement l'état rectal plein, partiellement rempli et vide. Une pdf de gaussiennes 2D de la fonction de mouvement rectal et d'épaisseur de la paroi rectale (2D pdf M&TW), en tant que modèle produit-mélange de la pdf du mouvement rectal (pdf M) et de l'épaisseur de la paroi rectale(pdf TW) a été développé à partir de données publiées et expérimentales, respectivement, et présentée sous forme mathématique. L'utilisation des valeurs de corrélation entre les fonctions pdf M et pdf TW permet la démonstration numérique de la sensibilité des profils et des projections de 2D pdf M&TW.
