The radical approach of letting people disappear from sociology is understandably conflicting and has been awarded with only poor consideration in the context of nursing science. However, Luhmann's systems theory offers an instrument for the observation of social interaction, which can extend the reflection of nursing actions. The question nursing science needs to ask is „how is it observed?", not „what is observed?" (Luhmann 1992:153ff). This second-order observation - the observation of the observers - allows a reflection of nursing that may be more equitable to the description of nursing than the dominant substantive definitions yet: representing succession of what-observations (i.e. ICN 2002) or explaining nursing care with itself. Luhmann explicated the code of the medical treatment system with ill/healthy. This difference is unsuitable for the care system; the previously established codes for a nursing system are in need of revision. Die Systemtheorie von Niklas Luhmann ist als soziologische Theorie in vielen Disziplinen rezipiert worden und hat einige Irritationen erzeugt. Der radikale Ansatz, den Menschen aus der Soziologie verschwinden zu lassen, ist dabei nachvollziehbarerweise auf viel Widerspruch gestoßen und hat möglicherweise aus diesem Grund im Kontext der Pflegewissenschaft eine Aufmerksamkeit erhalten, die bestenfalls als randständig zu bezeichnen ist. Doch bietet die Systemtheorie Luhmanns ein Instrument zur Beobachtung sozialer Kommunikation an, welche die pflegewissenschaftliche Reflexion des Handlungsfelds ordnen kann. Nicht die Frage des „Was wird beobachtet?" steht im Vordergrund, sondern die Frage nach dem „Wie wird beobachtet?" (Luhmann 1992: 153ff). Diese Beobachtung zweiter Ordnung, also die Beobachtung der Beobachtenden, ermöglicht eine Reflexion pflegerischer Tätigkeiten, die der Beschreibung der Pflege angemessener ist, als die bisher dominierenden substanziellen Definitionen, die letztlich Aneinanderreihungen der Was-Beobachtungen darstellen (exempl. ICN 2002) oder Pflege durch Pflege erklären. Luhmann explizierte den Code des Systems der Krankenbehandlung mit krank/gesund.
