        The addition of molecular genetic testing can complement morphological and immunophenotypic evaluation in the pathological diagnosis of lymphoma. The gene that encodes myeloid differentiation factor 88 (MyD88), an adaptor protein involved in signal transduction, sustains a somatic missense mutation (MyD88 (L265P)) in at least 86% of cases of lymphoplasmacytic lymphoma (LPL) but is uncommon in cases of marginal zone lymphoma (MZL) and other common indolent lymphomas that may resemble LPL histopathologically. MyD88 (L265P) is reliably detectable by allele-specific PCR (AS-PCR) even in relatively small biopsy specimens. Although MyD88 (L265P) is also relatively prevalent in other lymphoma types, including primary extranodal diffuse large B-cell lymphoma, these are readily distinguishable from the indolent lymphomas by other, more conventional means. Therefore, detecting MyD88 (L265P) by AS-PCR may be especially useful in the pathological distinction of LPL from other indolent B-cell neoplasms.
        En pathologie, lors d’un diagnostic de lymphome, l’ajout d’une analyse génétique moléculaire peut apporter un complément aux évaluations morphologique et immunophénotypique. Le gène qui code pour le facteur de différenciation myéloïde 88 (MyD88), une protéine adaptatrice intervenant dans la transduction du signal, comporte une mutation faux-sens (MyD88 (L265P)) dans au moins 86 % des lymphomes lymphoplasmocytaires, mais rarement dans les lymphomes de la zone marginale et d’autres lymphomes indolents courants potentiellement similaires au lymphome lymphoplasmocytaire sur le plan histopathologique. La mutation MyD88 (L265P) est repérable de façon fiable par PCR spécifique d’allèles, même dans des échantillons biopsiques de taille relativement petite. Bien qu’il y ait également une prévalence relativement importante de la mutation MyD88 (L265P) dans d’autres types de lymphomes, parmi lesquels les lymphomes diffus à grandes cellules B extraganglionnaires primitifs, ces derniers sont facilement différentiables des lymphomes indolents par d’autres méthodes plus courantes. La détection de la mutation MyD88 (L265P) par PCR spécifique d’allèles peut donc s’avérer extrêmement utile en pathologie pour différencier les lymphomes lymphoplasmocytaires des autres tumeurs indolentes à cellules B.
