Nurses should become facilitators of the palliative care transition. However, data submitted to implementations during the transition are scattered in the literature. Objective: To map interventions implemented by nurses during the transition to palliative care. Review Method: Scoping review following the Joanna Briggs Institute methodology. One independent reviewer analyzed the relevance of the studies and extracted and synthesized data. Presentation and interpretation of results: Six studies were included in the review. The interventions implemented by nurses' concern communication, capacity, coping, emotions and continuity of care. Conclusion: The interventions implemented by nurses described in the studies differ among themselves. Further studies should be conducted to identify the interventions implemented by nurses during the transition to palliative care. O processo de transição de cuidados para os cuidados paliativos não deve ser subestimado. Os enfermeiros devem tornar-se agentes facilitadores da transição para cuidados paliativos. Contudo, os dados relativos às intervenções implementadas durante a transição encontram-se dispersos na literatura. Objetivo: Mapear intervenções implementadas pelos enfermeiros durante a transição para cuidados paliativos. método de revisão: Scoping review com base nos princípios preconizados pelo Joanna Briggs Institute. Um revisor independente realizou a análise de relevância dos artigos, a extração e síntese dos dados. Apresentação e interpretação dos dados: Seis estudos foram incluídos na revisão. As intervenções implementadas pelos enfermeiros dizem respeito à comunicação, capacitação, coping, emoções e continuidade de cuidados. Conclusão: As intervenções implementadas pelos enfermeiros descritas nos estudos diferem entre si.
