      ab: Objective: To evaluate the accuracy and reliability of OneCeph smartphone application for cephalometric analysis in comparison with AutoCAD computer software. Design: A Cross-sectional study. Materials and Methods: Pretreatment digital lateral cephalograms of 50 orthodontic patients (25 males, 25 females) were analyzed with OneCeph application and AutoCAD software (as a standard measure). Fifteen landmarks were marked on each cephalogram and 10 measurements indicating skeletal, dental, and soft tissue parameters from Steiner's analysis were selected and measured (five angular and five linear measurements). A paired t-test was used to assess systematic bias. While Intraclass correlation (ICC) and Bland-Altman plot with linear regression analysis were used to assess the agreement between the two methods. Results: There was an excellent reproducibility for the measurements with OneCeph application (ICC: 0.950-0.999) and AutoCAD software (ICC: 0.973-0.998). Paired t-test did not show statistically significant differences, except for the SNA and SN-MP angles (P < 0.05), but these were of no clinical importance. The ICC test revealed excellent agreements for all the measurements between the OneCeph and AutoCAD, with the values ranged between 0.974 and 0.994. The Bland-Altman plot showed that the size of error was within the acceptable range and the linear regression analysis revealed no proportional bias between the two methods except the upper and lower lip measurements. Conclusions: OneCeph smartphone application can be used with adequate precision and reliability for cephalometric analysis as that with AutoCAD software.
