Trauma is a general term that includes all internal and external injuries caused by previous violence, contusions, abrasions, wounds and fractures, are the most representative in the facial region. Objective. To know the incidence of maxillofacial trauma in patients who attended a trauma emergency hospital in Mexico City in the period from November 2000 to November 2021. Methodology. All the patients attended by the maxillofacial surgery specialty in one year were evaluated, from November 1, 2000 to November 30, 2001, at the Hospital de Urgencias Traumatologicas "Dr. José Manuel Ortega Domínguez" Delegation 3 Southeast of IMSS of Mexico City, to know the incidence of maxillofacial trauma: most frequent injuries, age, sex, cause and place of the trauma accident. Results. A total of 340 patients (100%); 234 men (68.82%) and 106 women (31.18%). 308 patients (90.58%) were attended on an outpatient basis with treatment under local anesthesia; and 32 (9.41%) were hospitalized for surgery under general anesthesia. The mean age was 32 years (± 17.25) (standard deviation) with age limits from two to 88 years. The most frequent patient injuries were: blunt trauma 149 (43.82%), fractures 136 (40%) and soft-tissue wounds 55 (16.18%). Of the fractures, the most frequent were: nasal 63 (46.32%), mandible 37 (27.21%), orbicular 23 (16.91%), malar zygomatic 5 (3.68%), Le Fort II 2 (1.47%), Fx. TMJ 2 (1.47%), maxillary 2 (1.47%) and dentoalveolar 2 (1.47%); and the sites where they occurred: public roads 205 (60.29%), home 60 (17.64%), workplaces 31 (9.12%), recreational places 20 (5.88%), schools 11 (3.24%), and others 13 (3.82%). The highest incidence was due to: assaults with aggression 84 (40.98%), car accidents 44 (21.46%), fights 42 (20.49%), falls from the supporting plane 32 (15.61%), blows with objects 2 (0.98%) and by stray bullet 1 (0.49%). Conclusions. The incidence of assaults with violence stands out, followed by automobile accidents (a cause that years ago occupied the first place). Social-cultural, geographic and occupational factors are considered in this study group, which differ from others due to their nature; it is expected that with preventive campaigns in the public sphere, improvement in road safety and technological development in automobiles (seat belts, air bags, ABS brakes, etc.) and with guidance on accident prevention and protection of the maxillofacial region by maxillofacial surgeons, the incidence will decrease. Traumatismo es un término general que comprende todas las lesiones internas y externas provocadas por una violencia anterior; las contusiones, abrasiones, heridas y fracturas, son las más representativas en la región facial. Objetivo. Conocer la incidencia de traumatismo maxilofacial de pacientes que acudieron a un hospital de urgencias traumatológicas en la Ciudad de México en el periodo de noviembre de 2000 a noviembre de 2021. Metodología. Se evaluaron todos los pacientes atendidos por la especialidad de cirugía maxilofacial en un año, del 1 de noviembre de 2000 al 30 de noviembre de 2001, en el Hospital de Urgencias Traumatológicas "Dr. José Manuel Ortega Domínguez" Delegación 3 Sureste del IMSS de la Ciudad de México, para conocer la incidencia del traumatismo maxilofacial: lesiones más frecuentes, edad, sexo, causa y lugar del accidente de los traumatismos. Resultados. Un total de 340 pacientes (100%); 234 hombres (68.82%) y 106 mujeres (31.18%). 308 pacientes (90.58%) fueron atendidos en forma ambulatoria, con tratamiento bajo anestesia local; 32 (9.41%) hospitalizados para intervención quirúrgica bajo anestesia general. La edad promedio fue de 32 años (± 17.25) (desviación estándar) con límites de edad de dos a 88 años. Las lesiones de los pacientes más frecuentes fueron: contusiones 149 (43.82%), fracturas 136 (40%) y heridas 55 (16.18%). De las fracturas, las más frecuentes fueron: nasales 63 (46.32%), mandibulares 37 (27.21%), orbitarias 23 (16.91%), arco cigomático cinco (3.68%), Le Fort II dos (1.47%), Fx. ATM dos (1.47%), maxilar dos (1.47%) y dentoalveolares dos (1.47%); los sitios donde ocurrieron: vía pública 205 (60.29%), hogar 60 (17.64%), centros de trabajo 31 (9.12 %), lugares de recreación 20 (5.88%), escuelas 11 (3.24%), y otros 13 (3.82%). La mayor incidencia se debió a: asaltos con agresión 84 (40.98%), accidentes automovilísticos 44 (21.46%), riñas 42 (20.49%), caídas del plano de sustentación 32 (15.61%), golpes con objetos dos (0.98%) y por bala perdida uno (0.49%). Conclusiones. Destaca en la incidencia: asaltos con violencia, seguidos por accidentes automovilísticos (causa que años atrás ocupaba el primer lugar). Se consideran factores socialesculturales, geográficos y de actividad ocupacional de este grupo de estudio, que difieren de la de otros por su naturaleza; se espera que con campañas preventivas en el ámbito público, mejora en la seguridad vial y desarrollo tecnológico en automóviles (cinturón de seguridad, air-bags, frenos ABS, etc.) y con orientación sobre prevención de accidentes y protección de la región maxilofacial por parte de los cirujanos maxilofaciales, disminuya la incidencia. |
