It occurs in 10 patients for every million inhabitants, more frequently in women. There are different variants of this disease, such as classic, bullous, pustular, vegetative, drug-induced and peristomal. The bullous variety begins in unusual topography such as face, extensor regions and hands, having a strong association with concomitant hematological diseases. The diagnosis is usually clinical and to confirm it a biopsy has to be performed; systemic corticosteroids are considered the first-line treatment. CLINICAL CASE: A 33-year-old male patient, who, after a trauma that caused loss of skin continuity, presented multiple blisters that left extensive exulcerations, treatment was started in another facility with antibiotics and surgical debridement, which caused the existing lesions to spread and increase in size. In our hospital, a diagnosis of bullous pyoderma, a bullous variety, was confirmed histopathologically. CONCLUSIONS: It is important to know the different presentations of the disease, in order to establish a timely diagnosis and avoid cases of associated morbidity and mortality. ANTECEDENTES: El pioderma gangrenoso es una dermatosis neutrofílica inflamatoria que puede tener una amplia variedad de manifestaciones clínicas. Afecta a 10 pacientes por cada millón de habitantes; con mayor frecuencia en mujeres. Existen distintas variantes de esta enfermedad, como la clásica, ampollosa, pustular, vegetante, inducida por fármacos y periestomal. La variedad ampollosa inicia en topografía poco habitual, como la cara, las regiones extensoras y las manos, teniendo una fuerte asociación con enfermedades hematológicas concomitantes. El diagnóstico suele ser clínico y se confirma con biopsia; los corticosteroides sistémicos son el tratamiento de primera línea. CASO CLÍNICO: Paciente masculino de 33 años, quien tras un traumatismo que ocasionó pérdida de la continuidad de la piel, manifestó múltiples ampollas que dejaban amplias exulceraciones, contra las cuales se inició tratamiento en otro centro con antibiótico y debridación quirúrgica, lo que provocó diseminación y aumento en tamaño de las lesiones ya existentes. En nuestro hospital se estableció el diagnóstico de pioderma gangrenoso variedad ampollosa, confirmado de manera histopatológica.
