        We herein report a case of an 80-year-old female patient who presented with a rare morphologic variant of acute promyelocytic leukemia (APL). Following a bone marrow biopsy, fluorescence in situ hybridization for PML-RARA fusion and cytogenetics confirmed the diagnosis of t(15:17)- rearranged APL. Although classic bilobed promyelocytes were seen, a significant subset of blasts (more than 30%) in the peripheral blood and aspirate demonstrated a high nuclear:cytoplasmic ratio, basophilic cytoplasm, and multiple cytoplasmic blebs. These blasts were akin to micromegakaryocytes morphologically. So-called hyperbasophilic variants of APL have been described in the past with similar features and are considered very rare. Such a case exemplifies the potential morphologic spectrum of leukemia. Increased recognition of hyperbasophilic, micromegakaryocyte-like blasts in APL may further elucidate therapeutic significance of such morphology at presentation.
        Nous présentons ici le cas d'une patiente de 80 ans atteinte d'une rare variante morphologique de la leucémie promyélocytaire aigüe (LPA). Après une biopsie de moelle osseuse, une hybridation in situ en fluorescence pour vérifier la fusion des gènes PML et RARA et une analyse cytogénétique ont permis de confirmer le diagnostic de LPA avec translocation t(15:17). Malgré la présence de promyélocytes classiques avec noyau d'aspect bilobé, un sous-groupe significatif de blastes (plus de 30 %) dans le sang périphérique et dans l'aspirat présentait un rapport nucléocytoplasmique élevé, un cytoplasme basophile et de multiples granulations cytoplasmiques. Sur le plan morphologique, ces blastes ressemblaient à des micromégacaryocytes. Ces variantes soidisant hyperbasophiles de la LPA ont été rapportées par le passé avec des caractéristiques similaires et sont considérées comme étant très rares. Un tel cas illustre le spectre morphologique potentiel de la leucémie. Une meilleure reconnaissance des blastes hyperbasophiles ressemblant à des micromégacaryocyte en présence de LPA peut éclairer davantage concernant la signification thérapeutique de telles caractéristiques morphologiques au moment de l'évaluation initiale.
