        The treatment of haemophilia is based on the administration of the defi cient clotting factor (FVIII for haemophilia A and FIX for haemophilia B). One of the main complications is that the administration of factor concentrates can trigger production of inhibitory antibodies targeting the exogenous FVIII or FIX; this response neutralises the activity of the factors which have been administered, and therefore represents the most important complication of replacement therapies for patients with haemophilia. In patients who develop inhibitors, it becomes more diffi cult to prevent and treat bleeding episodes, with an increased risk of severe bleeding, and a greater likelihood of developing severe arthropathy with musculoskeletal sequelae. This leads to a reduction in quality of life for sufferers of haemophilia. In recent years, we have seen unprecedented advances in the management of patients with haemophilia, particularly in patients with haemophilia presenting with inhibitors. However, production of inhibitors continues to be a serious complication that impacts on how these patients are managed. This paper presents a clinical case of a patient with severe haemophilia A with inhibitors who underwent treatment with emicizumab for both eliminating this inhibitor and for simultaneous prophylaxis of bleeding events.
        El tratamiento de la hemofi lia se basa en la administración del factor defi citario (FVIII en el caso de la hemofi lia A y FIX en el caso de la hemofi lia B). Una de las principales complicaciones se debe a que la administración de concentrados de factor puede desencadenar la aparición de anticuerpos inhibidores dirigidos frente al FVIII o FIX exógenos, que neutralizan la actividad del factor administrado, y que supone la complicación más importante del tratamiento sustitutivo en pacientes con hemofi lia. El desarrollo de un inhibidor conlleva una mayor difi cultad para prevenir y tratar los episodios hemorrágicos, aumenta el riesgo de hemorragias graves y predispone al desarrollo de artropatía grave con repercusión musculoesquelética. Todo ello genera un empeoramiento de la calidad de vida de los pacientes hemofílicos. En los últimos años hemos asistido a un avance sin precedentes en el tratamiento de los pacientes con hemofi lia, sobre todo en el caso de los pacientes con hemofi lia e inhibidor. Sin embargo, la aparición de un inhibidor continúa siendo una seria complicación que condiciona el manejo de estos pacientes. En el presente trabajo se presenta el caso clínico de un paciente con hemofi lia A grave e inhibidor, al cual se le hizo una erradicación de este inhibidor y se trató simultáneamente la profi laxis de los eventos hemorrágicos con emicizumab.
