The most frequent is the ulcerative, characterized by rapidly evolving painful ulcers with weakened edges and peripheral erythema, mainly on the extremities and trunk. About half of the cases are associated with systemic diseases, such as rheumatoid arthritis, inflammatory bowel disease and hematologic diseases. CLINICAL CASE: A 59-year-old male patient with an 18-year history of arthropathy, misdiagnosed as gouty arthritis, currently suffering from arthritis mutilans. He was diagnosed with mutilating rheumatoid arthritis in 2021. He started 8 years ago with dermatosis on the external face of the lower third of the right leg with a papule that presented growth and subsequent ulceration; three months later, the same type of injury appeared in the lower third of the left leg, with rapid growth and intense pain. He was diagnosed with pyoderma gangrenosum. Surgical debridement of the slough was performed and management with prednisone 5 mg per day and Ig IV 1 g/kg was started; subsequently methotrexate 15 mg per week was added for treating the underlying condition. CONCLUSIONS: It is important to consider the association of pyoderma gangrenosum with other systemic diseases, which will allow us to reach a timely and accurate diagnosis of both and to avoid sequelae that impact the quality of life of patients. Immune globulin therapy is an effective treatment in extensive or refractory cases. ANTECEDENTES: El pioderma gangrenoso es una dermatosis neutrofílica rara que se divide en las siguientes categorías clínicas: ulcerosa, ampollosa, pustular y vegetante y una topográfica: periestomal. La más frecuente es la ulcerativa, caracterizada por úlceras dolorosas de rápida evolución, con bordes debilitados y eritema periférico, principalmente en las extremidades y el tronco. Aproximadamente la mitad de los casos se asocian con enfermedades sistémicas, como artritis reumatoide, enfermedad inflamatoria intestinal y enfermedades hematológicas. CASO CLÍNICO: Paciente masculino de 59 años de edad, con antecedente de artropatía de 18 años de evolución, mal diagnosticado como artritis gotosa, actualmente cursando con artritis mutilante. Se diagnosticó artritis reumatoide mutilante en 2021. Inició hacía 8 años con dermatosis en la cara externa del tercio inferior de la pierna derecha con una pápula que mostró crecimiento y posterior ulceración; 3 meses después apareció el mismo tipo de lesión en el tercio inferior de la pierna izquierda, con crecimiento rápido y dolor intenso. Se diagnosticó como pioderma gangrenoso. Se realizó debridación quirúrgica de esfacelo y se inició tratamiento con 5 mg de prednisona al día, Ig IV 1 g/kg; posteriormente se añadieron 15 mg de metotrexato a la semana para tratar el padecimiento de base. CONCLUSIONES: Es importante tener en cuenta la asociación de pioderma gangrenoso con otras enfermedades sistémicas, lo que nos permitirá llegar al diagnóstico oportuno y certero de ambas y evitar secuelas que repercutan en la calidad de vida de los pacientes.
