| Sumario: | Ulcerative colitis is the most common inflammatory bowel disease clinically conditioned by an inflammation of the colonic mucosa and rectum, which causes ulcerations, diarrhea, blood in the stool and abdominal pain. Its etiology is still unknown, but it is hypothesized about an alteration of the immune response to specific stimuli. It’s classified according to its severity, extension and colonic lesions, likewise its classification allows to guide the management, which aims to achieve a remission without the use of steroids, using 5-ASA and biological drugs; being necessary, in some cases, surgery. The most associated complications are toxic megacolon and, in the long term, the increased risk of colorectal cancer and in operated patients the risk of pouchitis. La colitis ulcerativa crónica idiopática es la enfermedad inflamatoria intestinal más común condicionada clínicamente por inflamación de la mucosa colónica y del recto, lo que ocasiona ulceraciones, diarrea, sangre en heces y dolor abdominal. Su causa se desconoce, pero se hipotetiza sobre una alteración de la respuesta inmunitaria a estímulos específicos. Se clasifica de acuerdo con su gravedad, extensión y lesiones colónicas, asimismo, su clasificación permite pautar el manejo, que tiene como meta lograr la remisión sin administración de esteroides, prescribiendo 5-ASA y fármacos biológicos; en algunos casos es necesario realizar cirugía. Las complicaciones más asociadas son el megacolon tóxico en agudo y, a largo plazo, el riesgo mayor de cáncer colorrectal y en pacientes operados el riesgo de pouchitis. |
