        AIM: This study examined a multistate model with correlated frailty between CKD transitions by fitting a model with a bivariate gamma distribution. Methodology: By incorporating as a frailty term, correlated multistate modeling was able to account for the dependence between moderate and severe CKD in an analysis of kidney dysfunction progression data from 153 patients. Therefore, frailty and correlated-frailty multistate models were used to evaluate the role that significant prognostic factors play in disease progression. Results: Hypertension and diabetes have a threefold and fourfold increased risk of progressing from state mild to moderate CKD in both models respectively, covid-19 has a significant risk of transitioning from mild to moderate and severe CKD state in both models. Conclusion: The underlying dynamics of CKD can be better understood with the help of multistate models, which do this by analyzing the subjects' entire event histories. Under the hypothesis of a positive correlation between the frailties for the two exits from mild CKD, this evidence supports the superiority of a model fit with a bivariate gamma distribution.
        Objetivo: Este estudio examinó un modelo multiestado con fragilidad correlacionada entre transiciones de ERC ajustando un modelo con una distribución gamma bivariante. Metodología: Mediante la incorporación como término de fragilidad, el modelado multiestado correlacionado fue capaz de dar cuenta de la dependencia entre la ERC moderada y grave en un análisis de datos de progresión de la disfunción renal de 153 pacientes. Por lo tanto, se utilizaron los modelos frailty y multiestado correlacionado-frailty para evaluar el papel que juegan los factores pronósticos significativos en la progresión de la enfermedad. Resultados: La hipertensión y la diabetes triplican y cuadruplican el riesgo de progresión del estado leve a moderado de ERC en ambos modelos respectivamente, el covid-19 tiene un riesgo significativo de transición del estado leve a moderado y grave de ERC en ambos modelos. Conclusiones: La dinámica subyacente de la ERC puede comprenderse mejor con la ayuda de modelos multiestado, que lo hacen analizando el historial completo de eventos de los sujetos. Bajo la hipótesis de una correlación positiva entre las fragilidades para las dos salidas de la ERC leve, esta evidencia apoya la superioridad de un modelo ajustado con una distribución gamma bivariante.
