Sumario:The incidence of periprosthetic humeral fractures (PPHF) is trending upwards subject to rising total numbers of shoulder arthoplasties and the demographic ageing. Nonetheless, there is no consensus neither about classification nor the optimal treatment algorithm. Nondisplaced fractures of the tubercles or distally to the stem can be treated non-operatively when the arthroplasty is well-fixed. Conservative treatment in general though is associated with a high percentage of non-unions. For stable implants with sufficient bone stock, a fixed angle plate osteosynthesis can be performed, which yields the highest union rates overall. Whenever the stem is loose and/or the bone quality is poor, a revision arthroplasty has to be performed. In general, treatment of PPHF is a very complex situation which requires surgeons to take the fracture, the integrity of the stem and individual circumstances into consideration.
Die Inzidenz periprothetischer Humerusfrakturen (PPHF) nimmt aufgrund des demografischen Wandels und der zunehmenden Implantation von Schulterprothesen stetig zu. Weder über die Einteilung noch über die optimale Versorgungsstrategien dieser Frakturen besteht allerdings ein Konsens. Während Frakturen im Bereich der Tuberkel und distal des Prothesenschaftes auch konservativ behandelt werden können, sofern der Schaft stabil sitzt, zeigen sich für die übrigen Frakturentitäten hohe Pseudarthroseraten unter nicht-operativer Therapie. Nachdem prä- oder intraoperativ ein gelockerter Prothesenschaft festgestellt wurde, wird weitgehend ein Wechsel des Implantates auf einen Langschaft, der die Fraktur suffizient überbrückt, empfohlen. Bei einer festsitzenden Prothese kann bevorzugt die osteosynthetische Versorgung mit winkelstabilen Plattensystemen erfolgen, da hier einheitlich über die höchste Frakturheilungsrate berichtet wird. Grundsätzlich handelt es sich bei der Frakturversorgung von PPHF jedoch um einen hochkomplexen Sachverhalt, der prä- und intraoperativ eine sorgfältige Abwägung verlangt und neben der Frakturlokalisation insbesondere die Stabilität der Prothese, aber auch individuelle Aspekte, berücksichtigen sollte.
