| Sumario: | End-of-life (EOL) care in critical settings poses ethical and logistical challenges, emphasizing the need for standardized processes to ensure patient comfort. This quality improvement (QI) project implemented and evaluated a palliative ventilator withdrawal algorithm, incorporating a validated and reliable objective tool to assess dyspnea. We used the Consolidated Framework for Implementation Research to examine facilitators, challenges, and lessons learned to inform future hospital-wide implementation. • Guided by the Consolidated Framework for Implementation Research, facilitators and challenges, along with corresponding solutions, were identified during the pilot implementation of a ventilator withdrawal algorithm. • Key insights include (1) stakeholder buy-in and organizational alignment; (2) use of workflow-focused, scalable implementation; (3) teamwork and interdisciplinary collaboration; and (4) continuous quality improvement. • These facilitators, challenges, and lessons learned offer a practical roadmap for enhancing end-of-life care while increasing the success and sustainability of quality improvement initiatives.
