It is associated with systemic diseases, mainly from autoimmune and autoinflammatory etiology Regarding its paraneoplastic origin, it has been described following solid organ or hematologic neoplasia. CLINICAL CASE: A 61-year-old female patient with a history of diffuse large B-cell lymphoma nodal germinal center treated with doxorubicin, cyclophosphamide, vincristine, prednisone, and rituximab. In January 2022, she developed retroperitoneal lymph node metastases and infiltration of the left iliac fossa skin. One month later, she developed a localized dermatosis on the trunk of the breasts and abdomen, characterized by painful irregular-border erythematous-violaceous well-defined ulcers with eschar and necrosis, with the appearance of shell valves. Skin biopsy reported necrosis of epidermis and dermis with a predominant inflammatory infiltrate of polymorphonuclear cells, which led to the diagnosis of paraneoplastic pyoderma gangrenosum associated with diffuse large B-cell lymphoma nodal germinal center. CONCLUSIONS: Paraneoplastic pyoderma gangrenosum associated with large B-cell lymphoma is rare, it can be developed in patients undergoing chemotherapy, so in the presence of new skin lesions, other etiologies such as pyoderma gangrenosum must be considered, even in patients receiving chemotherapy with a history of skin infiltration. ANTECEDENTES: El pioderma gangrenoso es una dermatosis neutrofílica poco común que afecta principalmente a mujeres de mediana edad y se caracteriza por úlceras dolorosas con borde socavado y eritema periférico. Se asocia con enfermedades sistémicas, principalmente de origen autoinmunitario y autoinflamatorias. En cuanto a su origen paraneoplásico, se ha descrito secundario a neoplasia de órgano sólido y hematológica. CASO CLÍNICO: Paciente femenina de 61 años con antecedente de linfoma difuso de células B grandes, tipo centrogerminal ganglionar, tratado con doxorrubicina, ciclofosfamida, vincristina, prednisona y rituximab. En enero de 2022 tuvo metástasis a ganglios retroperitoneales e infiltración a la piel de la fosa iliaca izquierda. Un mes después manifestó una dermatosis localizada en el tronco, las mamas y el abdomen, constituida por múltiples úlceras dolorosas con escaras y necrosis, de borde irregular, eritematovioláceo, bien definidas con apariencia de valvas de concha. El estudio de la biopsia de piel reportó necrosis de epidermis y dermis con infiltrado inflamatorio predominante de neutrófilos, por lo que se estableció el diagnóstico de pioderma gangrenoso paraneoplásico asociado con linfoma difuso de células B grandes centro-germinal ganglionar.
