CLINICAL CASE: A 22 year-old male patient with moderate severe acne vulgaris on baseline developed acne fulminans and pyoderma gangrenosum one month later after starting systemic therapy with oral isotretinoin; the first dermatosis mainly affected the upper trunk on both sides, consisting of multiple ulcer-crusted plaques with an erythematous base, irregular edges, well-defined with scattered pustules, papules, and nodules. The second dermatosis affected the lower limbs at the level of the middle and lower thirds on all their faces, consisting of ulcers with a dirty bottom, erythematousviolaceous edges, irregular and well-defined with abundant sero-sanguineous shores with purulent discharge and flaccid blisters of serous content on erythematous skin; accompanied by fever and general state attack. Both dermatoses were characterized by an overexpression of interleukin (IL)-1b, IL-17, tumor necrosis factor (TNF)-α, and multiple chemokines, through inflammasome activation and neutrophil migration. The initial treatment was based on systemic steroid. When the complications due oral isotretinoin therapy resolved and no relapse was observed, the reduction of the oral steroid was started and management with oral tetracycline was reinitiated. CONCLUSIONS: Pyoderma gangrenosum and acne fulminans can be associated with oral isotretinoin therapy, although these are rare adverse events and even more so simultaneously. ANTECEDENTES: La isotretinoína juega un papel importante en la aparición de dermatosis neutrofílicas, incluido el síndrome de Sweet y el pioderma gangrenoso en pacientes con lesiones graves de acné. CASO CLÍNICO: Paciente masculino de 22 años con acné vulgar moderado a severo, que manifestó acné fulminante y pioderma gangrenoso un mes posterior al inicio del tratamiento sistémico con isotretinoína oral; la primera dermatosis afectaba principalmente el tronco superior en ambas caras, constituida por múltiples placas úlcero-costrosas de base eritematosa, bordes irregulares, bien definidos con pústulas, pápulas y nódulos dispersos. La segunda dermatosis afectaba los miembros inferiores en el tercio medio e inferior en todas sus caras, constituida por úlceras de fondo sucio, de bordes eritematovioláceos, irregulares y bien definidos con abundantes costras serosanguíneas con secreción purulenta y ampollas flácidas de contenido seroso sobre piel eritematosa, acompañadas de fiebre y ataque al estado general. Ambas enfermedades se caracterizaban por sobreexpresión de interleucina (IL)-1b, IL-17, factor de necrosis tumoral (TNF)-α y múltiples quimiocinas, a través de la activación del inflamasoma y migración de neutrófilos. El tratamiento inicial fue con esteroide sistémico; al aliviarse las complicaciones derivadas de la isotretinoína oral y no observar recaída, se inició la reducción del esteroide oral y tratamiento con tetraciclina oral.
