Methods: A 34-year-old male patient's cone beam computed tomography (CBCT) images were investigated. Results: Ten cusps and a pyramidal pulp chamber were observed on tooth 47. Mimics 21.0 and Geomagic 21.0 software were used for three-dimensional (3D) reconstruction of the deformed tooth. Moreover, teeth 16, 17, 26 and 27 were classified as hypotaurodontism based on CBCT analysis. Conclusion: Simultaneous occurrence of supernumerary cusps and taurodontism in one tooth or patient is rare.
