Objectives: To present 2 cases of midface reconstruction using a free scapula tip flap over a 4-month period. Materials and Methods: Report of 2 clinical cases. Results: Two clinical cases are reported with malignant maxillary sinus neoplasms that underwent maxillectomies with zygoma and pre-maxilla resection, with successful microvascular reconstruction using a free scapula tip flap. Discussion: The scapula tip flap for midface reconstruction presents adequate aesthetic and functional results, allowing reconstruction of a wide variety of defects due to its great versatility and availability of soft tissue and bone. This flap should be taken into consideration when reconstructing this critical area in the head and neck. Introducción: La reconstrucción del tercio medio facial y maxilar corresponde a un desafío mayor por la complejidad de esta región. Existen distintos técnicas de reconstrucción con el fin de lograr un resultado funcional y estético adecuado. Objetivos: Presentar 2 casos de reconstrucción de tercio medio facial a través del colgajo libre de punta de escápula en un período de 4 meses. Materiales y Métodos: Reporte de 2 casos clínicos. Resultados: Se reportan casos de 2 pacientes con neoplasias malignas de seno maxilar en quienes se realizó una maxilectomía con resección de malar o premaxila, con reconstrucción microvascular exitosa mediante colgajo libre de punta de escápula. Discusión: El colgajo de punta de escápula para la reconstrucción del tercio medio facial presenta adecuados resultados tanto estéticos como funcionales, permitiendo reconstruir una gran variedad de defectos debido a su gran versatilidad y disponibilidad de tejido blando y óseo.
