| Sumario: | Sleep-Disordered Breathing (SDB) is highly prevalent and underdiagnosed in the pediatric population. The gold-standard test for its diagnosis and categorization is polysomnography (PSG), though access to it is limited in our setting. Polygraphy (PG) is a more accessible and versatile alternative that helps to reduce the diagnostic gap of SDB at a local level. In these recommendations, the authors –members of the Chilean Society of Pediatric Pulmonology (SOCHINEP), the Chilean Society of Respiratory Diseases (SER-Chile), and the Chilean Society of Sleep Medicine (SOCHIMES)– aim to establish guidelines for the use of PG in pediatrics. Technical and practical aspects and interpretation are discussed in depth, including specific comments on special populations such as children under 3 months, hospitalized in critical units, patients with neuromuscular disease, obesity, craniofacial malformations, and chronically ventilated. Thus, these guidelines establish recommendations applicable throughout Chile for the use of PG as a reliable and standardized tool in various pediatric clinical settings. Los Trastornos Respiratorios del Sueño (TRS) son muy prevalentes y se encuentran subdiagnosticados en la población pediátrica. La prueba de referencia para el diagnóstico y su categorización es la polisomnografia (PSG), cuyo acceso es limitado en nuestro medio. Por esto, la poligrafia (PG) es una alternativa más accesible y versátil, que contribuye a disminuir la brecha diagnóstica de los TRS a nivel local. En estas recomendaciones, los autores, miembros de la Sociedad Chilena de Neumología Pediátrica (SOCHINEP), Sociedad Chilena de Enfermedades Respiratorias (SER-Chile) y la Sociedad Chilena de Medicina del Sueño (SOCHIMES) se plantearon como objetivo establecer directrices para el uso de PG en pediatría. Se profundiza sobre aspectos técnicos, aspectos prácticos e interpretación; incluyendo comentarios específicos sobre poblaciones especiales como menores de 3 meses, hospitalizados en unidades críticas, pacientes con enfermedades neuromusculares, obesidad, malformaciones craneo-faciales y ventilados crónicos. De esta forma, se establecen recomendaciones aplicables para todo Chile en el uso de la PG como una herramienta confiable y estandarizada en diferentes escenarios clínicos en pediatría. |
