Sumario:Objective: This article introduces an alternative to the classic flowable injection technique to enhance esthetic outcomes and describes its application for the restoration of six maxillary anterior teeth in a young patient. Clinical Considerations: An esthetically enhanced flowable injection technique was applied to six maxillary anterior teeth after a fully digital treatment plan. A virtually guided dentin cutback model was created from the digital full wax up. Clear polyvinyl siloxane indices were created from three different 3D‐printed models: one from the dentin cutback model of all teeth, one from the enamel model with every other tooth waxed up completely in an alternating sequence, and the third one from the enamel wax up model of all teeth. The two different enamel models are only necessary when multiple anterior teeth are restored to simplify interproximal contouring and control. The multilayer approach enhances esthetic outcomes of direct resin‐based flowable composite restorations without any or only minimal tooth preparation. Conclusions: The multilayer injection technique facilitates predictable transfer of the digitally planned dentin and enamel tooth structure shapes and volumes to the patient's mouth to fabricate highly esthetic and minimally invasive direct restorations with flowable resin‐based composites. Clinical Significance: The precise transfer of the digital tooth and/or smile design with the updated flowable injection technique allows the clinician to fabricate esthetic restorations in a most predictable and tooth‐structure‐preserving manner.
