        Problem: Several challenges are associated with the transition of older adults from a long-term care facility (LTCF) to the emergency department (ED). Nurses play an important role in LTCFs and EDs during this transition. Objective: To describe the perceptions of nurses and managers in relation to this transition. Methods: This study used a sequential mixed methods design. An online questionnaire was distributed to LTCF and ED nurses during Phase 1 (quantitative). During Phase 2 (qualitative), semi-structured interviews were conducted with management in these two settings. Descriptive statistics were used as well as t-tests for the quantitative data. Thematic analysis was used for the qualitative data. Results: After the analysis of the questionnaire's responses (n = 38), the nurses from LTCFs and EDs had similar perceptions and considered the transition of older adults from the LTCF to the ED as being ineffective, and the inter-institutional communication was not efficient. Following the interviews conducted with managers (n = 7), four themes were identified, reflecting the limitations of LTCFs in caring for the older adults, the obstacles associated with the transfer of information, the consequences of an ED stay for older adults, and the contribution of caregivers. Conclusion: Four key conclusions can be drawn: 1. this transition is influenced by the LTCF material and human resources; 2. the transfer of information is inefficient; 3. the ED environment is not adapted for the needs of older adults; and 4. the caregivers' integration is important but does present with challenges.
        Problématique : La transition des aînés des établissements de soins de longue durée (ÉSLD) vers l'urgence présente plusieurs défis. Lors de cette transition, le personnel infirmier y détient un rôle important tant dans les ÉSLD qu'à l'urgence. Objectif : Décrire les perceptions du personnel infirmier et cadre de l'Outaouais (Québec) à l'égard de cette transition. Méthode : Cette étude a employé un devis mixte séquentiel explicatif. À la phase 1 (quantitative), un questionnaire en ligne a été distribué au personnel infirmier des ÉSLD et des urgences. À la phase 2 (qualitative), des entrevues individuelles semi-dirigées ont été menées auprès du personnel cadre de ces milieux. Des analyses statistiques ont été effectuées et des tests-t sur les données quantitatives. Les données qualitatives ont été analysées à l'aide d'une analyse thématique. Résultats : À la lumière des réponses provenant du questionnaire (n = 38), le personnel infirmier des ÉSLD et des urgences partageaient des perceptions similaires et considéraient que cette transition se déroulait inefficacement et que la communication interétablissements était inefficiente. À partir des entrevues menées avec le personnel cadre (n = 7), quatre thèmes ont été développés témoignant des limites des ÉSLD dans la prise en charge des aînés, des obstacles dans le transfert de l'information, des répercussions d'un séjour à l'urgence pour les aînés et de la contribution des personnes proches aidantes (PPA). Conclusion : Quatre constats sont tirés : 1) cette transition est influencée par les ressources humaines et matérielles des ÉSLD; 2) le transfert de l'information est inefficace; 3) l'environnement de l'urgence n'est pas adapté aux besoins des aînés; et 4) l'intégration des PPA est importante, mais présente des défis.
