This cross-sectional study was conducted with 182 mother-child pairs seen at a university hospital in Paraíba, aiming to investigate possible associations between maternal and neonatal variables and abnormal test results. Data collection included information on age, comorbidities, prematurity, congenital infections, and genetic syndromes. Only one case (0.5%) showed an altered result, with no statistically significant associations found with the analyzed variables. The low incidence of altered results limits the correlation analysis but does not diminish the importance of the test as a tool for early detection. The study reinforces the need for universal application of the test in newborns, even in the absence of evident risk factors, and highlights the importance of research with larger samples to deepen the understanding of the determinants of leukocoria. O Teste do Reflexo Vermelho é um exame essencial na triagem neonatal, por permitir a identificação precoce de alterações oculares graves, como a leucocoria. Este estudo transversal foi realizado com 182 pares mãe-filho atendidos em hospital universitário na Paraíba, com o objetivo de investigar possíveis associações entre variáveis maternas e neonatais e alterações no teste. A coleta de dados incluiu informações sobre idade, comorbidades, prematuridade, infecções congênitas e síndromes genéticas. Apenas um caso (0,5%) apresentou resultado alterado, não sendo observadas associações estatisticamente significativas com as variáveis analisadas. A baixa incidência de alterações limita a análise de correlações, mas não diminui a importância do exame como ferramenta de rastreio precoce.
