Despite surgical debridement and empirical antibiotic therapy, the patient developed new cutaneous lesions, fever, and hemodynamic instability, requiring invasive mechanical ventilation. During hospitalization, newly diagnosed hy perthyroidism and hypogammaglobulinemia due to IgG deficiency -findings consistent with common variable immunodeficiency- were documented. The absence of microbiological isolation, poor response to conventional therapeutic measures, and the emergence of new ulcers at sites of minimal trauma raised the suspicion of pyo derma gangrenosum, which was subsequently supported by clinical criteria and histopathological findings on skin biopsy. Early recognition of this entity is essential to prevent iatrogenic interventions, initiate timely immunomodulatory treatment, and reduce associated morbid ity and mortality. This case highlights the importance of considering pyoderma gangrenosum in the differential diagnosis of atypical cutaneous ulcers, particularly in patients with underlying immune dysfunction. Con el propósito de describir las características clíni cas y los desafíos diagnósticos del pioderma gangrenoso, se presenta el caso de una mujer de 39 años que consul tó por lesiones ulceradas en miembros inferiores y ante brazo izquierdo, inicialmente secundarias a mordeduras de perro con una semana de evolución. A pesar del des bridamiento quirúrgico y la antibioticoterapia empírica, la paciente desarrolló nuevas lesiones cutáneas, fiebre e inestabilidad hemodinámica, lo que motivó el reque rimiento de ventilación mecánica invasiva. Durante su internación, se diagnosticó hipertiroidismo de novo y se constató una hipogammaglobulinemia por déficit de IgG, hallazgo compatible con inmunodeficiencia común variable. La ausencia de aislamiento microbiológico, la escasa respuesta a las medidas terapéuticas conven cionales instauradas y la aparición de úlceras en sitios de mínima injuria orientaron la sospecha de pioderma gangrenoso, diagnóstico que fue respaldado por criterios clínicos y los hallazgos histopatológicos en biopsia cutá nea obtenidos. El reconocimiento temprano de esta en tidad es crucial para evitar intervenciones iatrogénicas, instaurar un tratamiento inmunomodulador oportuno y reducir la morbimortalidad asociada.
