        Introduction. Frailty is common in older adults and is associated with worse outcomes, including cardiovascular disease (CVD). However, limited data exist on its link to cardiovascular risk (CVR) in primary prevention and lipid target achievement. Objective. To assess the associations between frailty, measured by the 5-item FRAIL questionnaire, and estimated CVR, as well as LDL (cLDL) and non-HDL (cNoHDL) cholesterol goal attainment, in older adults without prior CVD. Methods. Secondary analysis of adults aged ≥60 years from the SABE Colombia study (n = 23,694). Frailty status was classified as robust, prefrail, or frail. CVR and lipid goal attainment were assessed via odds ratios (ORs) and regression models. Results: A total of 1,270 individuals were included; 22,146 were excluded because of missing CVR data, and 278 were excluded because of prior CVD. Frailty status: 48.58% robust, 48.35% prefrail, and 3.07% frail. cLDL goal achievement ranged from 0.72% to 3.93% according to the Framingham and ASCVD 2013 equations, respectively. Using the SCORE2 equation, cNoHDL goal achievement was 9.60%. Frailty was associated with a high CVR category with an OR of 5.53 (95% CI 2.05– 14.89, ASCVD 2013) and a very high CVR category with an OR of 12.05 (CI 2.77–52.39, SCORE2). Frailty was not linked to lipid target attainment. Conclusions. Over half of the participants had prefrailty or frailty. Frailty was associated with a higher CVR but not with lipid goal achievement, underscoring the need for targeted interventions in this vulnerable group.
        Introducción. La fragilidad es común en adultos mayores y se ha asociado con peores desenlaces, incluyendo enfermedad cardiovascular (ECV). Sin embargo, existe información limitada sobre su relación con el riesgo cardiovascular (RCV) en prevención primaria y el logro de metas lipídicas. Objetivo. Evaluar la asociación entre la fragilidad, medida mediante el cuestionario FRAIL de 5 ítems, y el RCV estimado, así como el logro de metas de colesterol LDL (cLDL) y colesterol no-HDL (cNoHDL) en adultos mayores sin ECV previa. Métodos. Análisis secundario de adultos ≥ 60 años años incluidos en el estudio SABE Colombia (n = 23,694). El estado de fragilidad se clasificó como robusto, prefrágil o frágil. El RCV y el cumplimiento de metas lipídicas se evaluaron mediante razones de odds ratios (OR) y modelos de regresión. Resultados. Se incluyeron 1,270 individuos; se excluyeron 22,146 por datos faltantes de RCV y 278 por ECV previa. Estado de fragilidad: 48,58% robustos, 48,35% prefrágiles y 3,07% frágiles. El cumplimiento de la meta de cLDL varió entre 0,72% a 3,93% según las ecuaciones de Framingham y ASCVD 2013, respectivamente. Empleando la ecuación SCORE2, el cumplimiento de meta de cNoHDL fue del 9,60 %. La fragilidad se asoció con una categoría alta de RCV con una OR de 5,53 (IC 95%: 2,05–14,89, ASCVD 2013) y con una categoría muy alta de RCV con una OR de 12,05 (IC 95%: 2,77–52,39, SCORE2). No se encontró asociación entre fragilidad y cumplimiento de metas lipídicas. Conclusiones. Más de la mitad de los participantes presentaron prefragilidad o fragilidad. La fragilidad se asoció con un mayor RCV, pero no con el logro de metas lipídicas, lo que resalta la necesidad de intervenciones dirigidas en este grupo vulnerable.
