It is due to a mutation in the gene that encodes cathepsin C and is characterized by palmoplantar hyperkeratosis, psoriasiform type plaques associated with severe periodontitis that generate exfoliation of temporary and permanent teeth at an early age. CLINICAL CASE: A 42-year-old female patient who presented delayed diagnosis after multiple infectious episodes and lost of the total dentition, being managed with prophylactic antibiotic therapy, transfer factor and systemic retinoid with adequate response. CONCLUSIONS: All patients with palmoplantar hyperkeratosis should be specifically questioned about a similar family history, chronic periodontitis, recurrent skin infections, and a history of abdominal abscesses in order to make a timely diagnosis to avoid recurrent infections and improve the quality of life of patients. ANTECEDENTES: El síndrome de Papillon-Lefévre es una genodermatosis tipo IV de tipo autosómico recesivo. Se estima que afecta a 1-4 por cada millón de habitantes y en 20 al 40% de los casos se asocia con consanguinidad. Se debe a una mutación en el gen que codifica a la catepsina C y se caracteriza por hiperqueratosis palmoplantar, placas de tipo psoriasiforme asociadas con periodontitis graves que generan exfoliación de las piezas dentales temporales y permanentes a temprana edad. CASO CLÍNICO: Paciente femenina de 42 años con retraso en el diagnóstico después de múltiples episodios infecciosos y pérdida de la dentición total. Recibió tratamiento con antibióticos profilácticos, factor de transferencia y retinoide sistémico con respuesta adecuada.
