Sumario:This article focuses on the implementation and evaluation of the Vascular-Perioperative medicine for Older People undergoing Surgery (POPS) model in managing Chronic Limb-Threatening Ischaemia (CLTI), a complex syndrome involving ageing, frailty, multimorbidity, and social vulnerability. It highlights that comprehensive geriatric assessment (CGA)-based co-management significantly improves clinical outcomes, reduces complications, length of stay, and mortality in older vascular surgery patients, including those undergoing emergency procedures. The study demonstrates that CGA-led care is both clinically effective and financially sustainable, advocating for its adoption as a standard practice across surgical specialties to better address the multifaceted needs of older patients. The article also emphasizes the need for broader inclusion of frail older adults in research and for outcome measures that reflect functional and cognitive status alongside traditional surgical metrics.
