Objective: To describe the immediate perioperative outcomes of pelvic exenterations performed at a high-complexity academic center. Methods: Retrospective cohort observational study. Patients who underwent pelvic exenteration between 2014 and 2023 were included. Demographic, operative, and postoperative variables were collected, with a descriptive analysis of perioperative outcomes. Results: A total of 25 pelvic exenterations were performed. The most frequent indication was rectal cancer, either primary (36%) or recurrent (32%). Bone resection was required in 44% of patients, and iliac vascular resection in 36%. Urinary reconstruction was performed in 80% and perineal flap closure in 40% of cases. Overall morbidity reached 80%, with major morbidity (Clavien-Dindo ≥ III) in 56%, and no perioperative mortality. R0 resection was achieved in 91.3% of cancer patients. Discussion: This study presents the perioperative outcomes of a high-complexity surgical procedure applied to various pelvic pathologies. Due to its technical demands, these findings emphasize the importance of centralization and specialized surgical training. Introducción: La exenteración pélvica es una intervención quirúrgica radical indicada en tumores localmente avanzados o recidivados de la pelvis. Su implementación requiere una infraestructura compleja, experiencia multidisciplinaria y un enfoque institucional sólido. Objetivo: Describir los resultados perioperatorios inmediatos de exenteraciones pélvicas realizadas en un centro académico de alta complejidad. Métodos: Estudio observacional de cohorte retrospectiva. Se incluyeron pacientes sometidos a exenteración pélvica entre 2014 y 2023. Se recolectaron variables demográficas, operatorias y postoperatorias, con análisis descriptivo de resultados perioperatorios. Resultados: Se realizaron 25 exenteraciones pélvicas. La indicación más frecuente fue cáncer de recto, primario (36%) o recurrente (32%). El 44% requirió resección ósea y el 36% de resección vascular ilíaco. El 80% requirió una reconstrucción urinaria y el 40% un cierre perineal con colgajo. La morbilidad global fue de un 80% y la morbilidad mayor (Clavien-Dindo ≥ III) fue del 56%, sin mortalidad operatoria. Se logró resección R0 en 91,3% de los casos. Discusión: Este trabajo presenta los resultados perioperatorios de un procedimiento quirúrgico de alta complejidad aplicado a diversas patologías pélvicas.
