| Sumario: | Aim: The aim of this study was to evaluate the association between the pucker sign and difficult cholecystectomy, postoperative morbidity, and the Nassar scale. Methods: A retrospective observational casecontrol study was conducted. Data from all LC performed at a single hospital between March 2022 and March 2023 were analyzed. Cases were divided into two groups based on the presence (complicated gallbladder lithiasis, CGL) or absence (uncomplicated gallbladder lithiasis, UGL) of complications. The UGL group was subdivided according to the presence or absence of the pucker sign. Difficult cholecystectomy was defined as partial cholecystectomy with or without cystic duct closure, transcystic bile duct instrumentation, primary duct closure, choledochoduodenostomy or hepaticojejunostomy. Results: A total of 425 LC procedures were performed during the study period. The pucker sign was observed in 31 patients within the UGL group (10%). A statistically significant association was found between the presence of the pucker sign and the need for complex procedures within the UGL group (p < 0.001). Additionally, the pucker sign was significantly associated with increased postoperative morbidity. Discussion: The pucker sign was significantly associated with increased surgical complexity, challenges in cystic duct closure, higher postoperative morbidity, and conversion to open surgery. Its occurrence was more prevalent among elderly patients, with no sex-based predominance. Conclusion: The recognition of the pucker sign should alert us to the possibility of a difficult cholecystectomy. Objetivo: El objetivo de este estudio fue relacionar el signo de fruncido con la colecistectomía difícil, la morbilidad postoperatoria y la escala de Nassar. Métodos: Estudio observacional retrospectivo de casos y controles. Se analizaron los datos de todas las colecistectomías laparoscópicas (CL) realizadas en un hospital entre marzo de 2022 y marzo de 2023. Los casos se dividieron en dos grupos según la presencia (LVNC) o ausencia (LVC) de complicación. El grupo LVC se subdividió según la presencia o ausencia del signo del fruncido. Se consideró colecistectomía difícil a las parciales con o sin cierre del cístico, la instrumentación del conducto biliar transcístico, cierre primario del conducto colédoco o de derivación bilio digestiva. Variables analizadas: diagnóstico de admisión, procedimientos quirúrgicos realizados, litiasis cística, dificultad para el cierre del muñón del cístico, Nassar, conversión, morbilidad postoperatoria e informe patológico. Resultados: Se realizaron 425 CL durante el período estudiado. El signo del fruncido estuvo presente en 31 del grupo LVC (10%). Se encontró una asociación estadísticamente significativa entre la presencia del signo de fruncido y la necesidad de procedimientos complejos dentro del grupo LVC (p < 0,001). Hubo una asociación significativa entre la morbilidad postoperatoria y el signo del fruncido. Discusión: La presencia del signo de fruncido se asoció con un procedimiento quirúrgico complejo, dificultad en el cierre del conducto cístico, morbilidad postoperatoria y conversión. Su presencia fue más común en pacientes mayores sin predilección de sexo. Conclusión: El reconocimiento del signo del fruncido debe alertarnos sobre la posibilidad de una colecistectomía difícil. |
