It requires intensive use of resources and is associated with high mortality, especially in blunt trauma. In Chile, data regarding the outcomes of this procedure are scarce. Objective: To describe short- and long-term outcomes of patients who underwent RT at a tertiary trauma center between 2018 and 2024. Methods: This was a retrospective study based on the trauma registry of Sótero del Río Hospital. Patients over 15 years old who underwent RT in the trauma bay were included. Demographic variables, injury mechanism, procedure indication, mortality, hospital stay, and complications were analyzed. Statistical analysis included Chi-square and Student’s t-tests, with p < 0.05 considered statistically significant. Results: A total of 78 patients who underwent RT were identified. Of these, 69.2% had penetrating trauma, with a 30-day survival rate of 14.8%. No survivors were recorded in the blunt trauma group. RT performed based on signs of life showed higher survival (15.3%) compared to RT based on CPR time (7.3%). Overall 30-day survival was 10.2%, and one-year survival was 9%. Conclusion: RT may be a valuable intervention in selected patients with penetrating trauma and signs of life. In resource-limited settings, its indication should be restricted to these cases to improve outcomes and optimize resource utilization. Introducción: La toracotomía de reanimación (TR) es una intervención quirúrgica de emergencia indicada en pacientes con trauma torácico grave, principalmente en paro cardiorrespiratorio (PCR) o en estado in extremis por shock hemorrágico. Su realización implica una utilización intensiva de recursos y está asociada a una alta mortalidad, especialmente en trauma cerrado. En Chile, existen escasos datos sobre los resultados de esta intervención. Objetivo: Describir los resultados a corto y largo plazo de pacientes sometidos a TR en un centro de trauma terciario, entre los años 2018 y 2024. Métodos: Estudio retrospectivo basado en el registro de trauma del Hospital Sótero del Río. Se incluyeron pacientes mayores de 15 años que recibieron TR en el box de trauma. Se analizaron variables demográficas, mecanismo de lesión, indicación del procedimiento, mortalidad, estancia hospitalaria y complicaciones. El análisis estadístico incluyó pruebas de Chi-cuadrado y t de Student, considerando p < 0,05 como significativo. Resultados: Se identificaron 78 pacientes sometidos a TR. El 69,2% presentó trauma penetrante, con una supervivencia a 30 días del 14,8%. No hubo sobrevivientes en el grupo con trauma cerrado. La indicación por signos de vida se asoció a una mayor supervivencia (15,3%) que la basada en tiempo de PCR (7,3%). La supervivencia global a 30 días fue del 10,2%, y a un año del 9%. Conclusión: La TR puede ser una intervención útil en pacientes seleccionados con trauma penetrante y signos de vida.
