        Early detection of critical congenital heart diseases is essential to reduce neonatal morbidity and mortality. At the primary care level, timely recognition of clinical warning signs, supported by tools such as pulse oximetry, allows for early referral and appropriate management. Objective: To describe strategies for the early identification and initial management of severe congenital heart diseases in newborns, aimed at primary healthcare professionals. Material and Method: Narrative review following the six domains of the Scale for the Assessment of Narrative Review Articles (SANRA) of the scientific literature published between 2015 and 2025, obtained from international databases. Clinical studies, clinical practice guidelines, and systematic reviews were selected based on their relevance and applicability to primary care settings. Results: Critical congenital heart diseases may present with respiratory distress, cyanosis, shock, and feeding intolerance. Diagnostic tools such as pulse oximetry and the hyperoxia test help differentiate cardiac from respiratory etiologies. Neonatal screening performed between 24 and 48 hours of life, complemented by a thorough physical examination, significantly improves early detection in resource-limited settings. Imaging studies may guide the differential diagnosis. Conclusions: Careful clinical assessment and the appropriate use of simple diagnostic tests are essential to reduce mortality associated with these heart conditions. Strengthening neonatal screening programs and training healthcare personnel should be considered a priority in primary care.
        La detección temprana de cardiopatías congénitas críticas es fundamental para reducir la morbimortalidad neonatal. En el primer nivel de atención, la identificación oportuna de signos clínicos de alarma, apoyada en herramientas como la oximetría de pulso, permite una referencia precoz y un manejo adecuado. Objetivo: Describir estrategias para la identificación precoz y el abordaje inicial de las cardiopatías congénitas graves en recién nacidos, orientadas al personal médico de atención primaria. Material y Método: Revisión narrativa siguiendo los seis dominios de la Scale for the Assessment of Narrative Review Articles (SANRA) de la literatura científica publicada entre 2015 y 2025, obtenida de bases de datos internacionales. Se incluyeron estudios clínicos, guías de práctica clínica y revisiones sistemáticas seleccionadas por su relevancia y aplicabilidad en la atención primaria. Resultados: Las cardiopatías congénitas críticas pueden manifestarse con dificultad respiratoria, cianosis, estado de choque y rechazo del alimento. Herramientas como la oximetría de pulso y la prueba de hiperoxia permiten distinguir la etiología cardíaca de la respiratoria. El screening neonatal entre las 24 y 48 horas de vida, complementado por un examen físico exhaustivo, mejora significativamente la detección precoz en entornos con recursos limitados. Los estudios de imagen orientan el diagnóstico diferencial. Conclusiones: La evaluación clínica cuidadosa y el uso adecuado de pruebas simples son esenciales para reducir la mortalidad asociada a estas cardiopatías. Fortalecer los programas de screening neonatal y capacitar al personal de salud constituye una prioridad en atención primaria.
