        Background.: Lifestyle Redesign® occupational therapy (LR-OT) originated in the Well Elderly studies as a preventive intervention for older adults, demonstrating positive health and cost outcomes. Although LR-OT later expanded to chronic condition management and inspired numerous programme adaptations for different contexts and populations, systematic reporting of intervention modifications remains scarce. This gap extends beyond OT, reflecting a broader movement in implementation science. Purpose.: This study applies the Framework for Reporting Adaptations and Modifications-Expanded (FRAME) to systematically document adaptations across four LR-OT programmes. Additionally, we examine fidelity-consistency by mapping programme components to the Well Elderly programme and core LR-OT elements. Methods. Four teams implementing adapted programmes—diabetic foot ulcer self-management, primary care chronic condition management, Remodeler sa Vie for French-Canadian older adults, and LR weight management—participated in a structured mapping exercise to align programmes with FRAME and LR-OT principle components. Results. Adaptation approaches varied from highly structured to loosely standardized methods. Despite numerous modifications, all programmes strongly aligned with LR-OT's core characteristics and domains. Conclusion. This study highlights LR-OT's adaptability across diverse contexts while maintaining fidelity to its foundational framework. Findings contribute to implementation research, providing a model for systematically documenting and characterizing adaptations made to client-centred, evidence-based OT programmes.
        Description. Le programme Lifestyle Redesign® ergothérapie (LR-ergo) est issu des Well Elderly studies. Il s'agit d'une intervention préventive pour les personnes âgées ayant démontré des résultats positifs en matière de santé et de coûts. Alors que le programme LR-ergo en est venu à s'étendre à la gestion des maladies chroniques et a inspiré de nombreuses adaptations pour différents contextes et populations, les modifications apportées à l'intervention sont rarement rapportées de manière systématique. Cette lacune va au-delà de l'ergothérapie et reflète un mouvement plus vaste dans la science de la mise en œuvre. But. La présente étude applique le cadre Framework for Reporting Adaptations and Modifications-Expanded (FRAME) pour documenter de manière systématique les adaptations apportées dans quatre programmes LR-ergo. Nous y examinons aussi la fidélité et la cohérence en faisant correspondre les composantes du programme au programme Well Elderly et aux éléments de base de LR-ergo. Méthodologie. Quatre équipes mettant en œuvre des programmes adaptés – autogestion de l'ulcère du pied diabétique, prise en charge des maladies chroniques en soins primaires, version franco-canadienne Remodeler sa vie pour les personnes âgées, gestion du poids – ont participé à un exercice structuré pour faire correspondre les programmes au cadre FRAME et aux principes de LR-ergo. Résultats. Les approches d'adaptation allaient d'approches très structurées à des méthodes standardisées de manière plus souple. Malgré de nombreuses modifications, tous les programmes sont restés très fidèles aux caractéristiques et domaines fondamentaux de LR-ergo. Conclusion. Cette étude met en évidence la possibilité d'adapter LR-ergo dans divers contextes tout en restant fidèles à son cadre fondamental. Les résultats contribuent à la recherche sur la mise en œuvre, fournissant un modèle pour documenter et caractériser systématiquement les adaptations apportées à des programmes d'ergothérapie centrés sur la clientèle et fondés sur les données probantes.
