      ab: Significance & Background: In today's complex healthcare environments, consistent and clearly defined leadership practices are critical to ensuring high-quality patient care, operational efficiency, and sustained staff and leader engagement. A recent span-of-control assessment conducted at an NCI-designated cancer center highlighted significant challenges related to role delineation among clinical leaders, particularly in teams with multiple levels of leadership. Overlapping responsibilities led to confusion around task ownership, escalation pathways, and accountability. This ambiguity not only creates operational inefficiencies but also contributes to staff and leader uncertainty, diminished confidence, and difficulty prioritizing work. Without clear role definitions, leaders may struggle with decision-making, delegation, and maintaining focus on strategic priorities--ultimately negatively impacting team performance and patient outcomes. Purpose: To develop and implement Leader Standard Work (LSW) across all nursing leadership roles--from Supervisor to Senior Director--by establishing structured responsibilities, clear role delineation, and clear escalation and support resources. Interventions: A multidisciplinary workgroup representing various levels and departments was convened. The group conducted a structured analysis of leadership functions identifying key responsibilities, areas of overlap, and inconsistencies in practice. Through collaborative discussion and iterative refinement, the workgroup developed a comprehensive LSW framework, including detailed task documentation, reference materials, and operational processes to promote consistency, accountability, and sustainability. Results: Thirteen LSW documents were developed to support consistent and accountable nursing leadership practices. These documents cover foundational leadership actions such as rounding, email etiquette, project ownership, and check-ins. A phased rollout with ongoing evaluation is planned beginning January 2026, with select LSWs introduced each quarter to support sustainable adoption and alignment. Discussion: The development of LSW documents marks a foundational step toward resolving leadership role ambiguity and enhancing operational clarity and efficiency. Standardizing key leadership behaviors promotes consistency, accountability, and strategic alignment across nursing leadership. The collaborative approach ensured the framework reflects diverse leadership realities. Review of initial LSWs has surfaced additional leadership tasks for future documentation, highlighting the dynamic nature of this work. While initially focused on nursing, discussions are underway to expand the LSW framework across the broader clinical enterprise to all leadership roles. This expansion would reinforce a unified leadership culture, reduce variability, strengthen organizational performance, and ultimately improve patient outcomes.
