Purpose: This initiative aims to standardize venous access assessment by implementing evidence-based guidelines and best practices to minimize risks for patients receiving chemotherapy and immunotherapy. A systematic approach ensures healthcare teams evaluate and discuss optimal peripheral IV versus central venous catheter placement based on individual patient conditions and treatment requirements before initiating therapy. Interventions: The evidence-based standardized venous access assessment intervention establishes a systematic, consistent approach to evaluating peripheral venous access prior to treatment, determining the most appropriate intravenous administration method. The team developed a standardized assessment plan to avoid unnecessary IV attempts by placing central lines when clinically indicated.
