| Sumario: | At a time of great concern regarding trends in morbidity and mortality from infections, and mindful of the limitations of our current approaches to combating them, this document sets out a structured programme of proposals aimed at improving, in the short and medium term, the survival of the most at-risk patients with severe infections. These are complementary proposals which, on the one hand, reinforce already established initiatives and, on the other, introduce some novel aspects to the approach to this complex problem. They focus on accelerating, standardising, and refining the clinical diagnosis of severe infection and its microbiological diagnosis and, above all, on improving the effectiveness of antimicrobial treatment, based on its immediate initiation with the most effective treatment available, and with an improved capacity to integrate innovation into all stages and processes. This programme is designed to be implemented within the framework of the Antimicrobial Stewardship Programmes (ASPs) (hence the name ASP-COMPLEX), and requires appropriate dissemination and surveillance policies, alongside a collaborative environment among the multiple stakeholders involved in delivering innovation (experts, ASP teams, healthcare administration, academia, the pharmaceutical industry, etc.). En un tiempo de gran preocupación por la evolución de la morbimortalidad de las infecciones, y conscientes de las limitaciones de nuestra manera de combatirlas, en este documento planteamos un programa estructurado de propuestas con la intención de mejorar, a corto y medio plazo, la supervivencia de los pacientes más vulnerables con infecciones graves. Se trata de propuestas de carácter complementario, que por un lado refuerzan iniciativas ya consolidadas y por otro replantean algunos aspectos novedosos en el abordaje de este complejo problema. Se centran en acelerar, generalizar y precisar mejor el diagnóstico clínico de la infección grave y su diagnóstico microbiológico y, sobre todo, en mejorar la efectividad del tratamiento antimicrobiano, con base en su inicio inmediato, con la mayor efectividad anticipable disponible, y con una capacidad mejorada para integrar la innovación en todos los pasos y procesos. Es un programa que ha de sustanciarse en el ámbito de los Programas de Optimización de Uso de Antibióticos (PROA) (de ahí el nombre de PROA COMPLEX), y que requiere unas adecuadas políticas de difusión y de vigilancia, además de un contexto de alianzas entre los múltiples agentes implicados en la provisión de innovación (expertos, PROA, administración, academia, industria farmacéutica, etc.). |
