        Introduction: Pleural effusion is an abnormal accumulation of fluid in the pleural space, and its correct classification into exudates or transudates is essential for diagnosis and treatment. The Light criteria are the laboratory standard for this classification, although they have limitations. Cholesterol in pleural fluid has been proposed as an alternative marker that could improve diagnostic accuracy. Objective: To evaluate the diagnostic utility of cholesterol in pleural fluid as a differential marker between exudative and transudative pleural effusions. Methods: An observational, cross-sectional, analytical study was conducted, including 80 pleural fluid samples collected at the Emergency Center of Metropolitan Lima. Cholesterol concentrations were determined using the enzymatic colorimetric method CHOD-PAP-Trinder, previously validated for pleural fluid. The ROC curve was used to establish the optimal cutoff point, and the sensitivity and specificity of the marker were calculated. Results: The ROC analysis revealed an area under the curve (AUC) of 0.83. The optimal cutoff point was 32 mg/dL, with a sensitivity of 88% and specificity of 74.5%. Chronic kidney disease was the most common pathology associated with pleural effusion in the studied sample (45%). Conclusion: Cholesterol in pleural fluid, measured using a validated technique, demonstrated adequate diagnostic performance to differentiate between exudative and transudative pleural effusions, with high sensitivity. Its incorporation into routine biochemical analysis could improve diagnostic accuracy in cases where the Light criteria are inconclusive
        Introducción: El derrame pleural es una acumulación anormal de líquido en el espacio pleural, y su correcta clasificación en exudados o trasudados es fundamental para el diagnóstico y tratamiento. Los criterios de Light son el estándar de laboratorio para esta clasificación, aunque presentan limitaciones. El colesterol en líquido pleural ha sido propuesto como un marcador alternativo que podría mejorar la precisión diagnóstica. Objetivo: Evaluar la utilidad diagnóstica del colesterol en líquido pleural como marcador diferencial entre derrames pleurales exudativos y trasudativos. Métodos: Se realizó un estudio observacional transversal analítico que incluyó 80 muestras de líquido pleural recolectadas en el Centro de Emergencia de Lima Metropolitana. Las concentraciones de colesterol se determinaron mediante el método enzimático colorimétrico CHOD-PAPTrinder, previamente validado para líquido pleural. Se utilizó la curva ROC para establecer el punto de corte óptimo y se calcularon la sensibilidad y especificidad del marcador. Resultados: El análisis ROC reveló un área bajo la curva (AUC) de 0.83. El punto de corte óptimo fue 32 mg/dL, con una sensibilidad del 88% y una especificidad del 74.5%. La enfermedad renal crónica fue la patología más frecuente de derrame pleural en la muestra estudiada (45%). Conclusión: El colesterol en líquido pleural, medido mediante una técnica validada, mostró un adecuado rendimiento diagnóstico para diferenciar entre derrames pleurales exudativos y trasudativos, con una alta sensibilidad. Su incorporación en el análisis bioquímico rutinario podría mejorar la precisión diagnóstica en casos donde los criterios de Light resulten poco concluyentes.
