| Sumario: | Background: Tinea incognito is a fungal infection modified by corticosteroids, leading to diagnostic confusion, atypical presentation, and recalcitrant symptoms. The over-the-counter availability of potent topical steroids in India has contributed to an epidemic of such "spoiled" cases. Case Summary: A 24-year-old male presented with extensive, poorly defined erythematous plaques on the thigh and knee, with intense itching and burning, following 4 months of inappropriate use of Clobetasol Propionate and Miconazole combination. Based on the totality of symptoms--including intense itching aggravated by heat of bed, craving for sweets, hot thermal state, and a history of recurrent skin rashes--an individualized homoeopathic approach was adopted. Intervention: Constitutional prescribing with Sulphur 30C (weekly), intercurrent Bacillinum 1M (fortnightly), and supportive Azadirachta indica 1x were administered alongside topical Sulphur Q and Calendula Q. Outcomes: Complete clinical resolution was achieved within 12 weeks, with restoration of skin texture and no recurrence at 6-month follow-up. Conclusion: This case demonstrates that homoeopathy can successfully reverse iatrogenic suppression and restore skin health by treating the "soil" (host susceptibility) rather than merely addressing the "seed" (fungal pathogen). It highlights the importance of anti-psoric prescribing and miasmatic intercurrents in managing steroid-modified dermatophytosis.
