        Introduction: recurrent urinary tract infections (rUTI) is defined as three episodes in six months or four in one year and is associated with an increased risk of renal scarring and damage. Objective: to identify risk factors associated with the development of renal scarring in pediatric patients with rUTI. Material and methods: this was a longitudinal, retrospective study of patients treated at a tertiary-level pediatric nephrology service. The following variables were analyzed: sex, age, nutritional status, isolated microorganism, kidney size, etiology of UTI. Results: forty-eight patients were included, 43 of whom were women, with a median age of 48 months; 89% had Escherichia coli infection. Thirty-nine percent presented with vesicoureteral reflux, 25% with urinary obstruction, and 14.5% with neurogenic bladder. Eightythree percent developed renal scarring and 62% with kidney damage, with a decrease in 47% of these cases. The only factor associated with renal scarring and damage was the detection of abnormalities on renal ultrasound. Conclusions: rUTI are associated with a high frequency of nephrotic scarring and decreased GFR. Identifying abnormalities on renal ultrasound is key to detecting patients at higher risk of kidney damage.
        Introducción: la infección de vías urinarias recurrente (IVUr) se define como tres episodios en seis meses o cuatro en un año, y se asocia con mayor riesgo de cicatrización y daño renales. Objetivo: identificar los factores de riesgo asociados al desarrollo de enfermedad cicatricial en pacientes pediátricos con IVUr. Material y métodos: estudio longitudinal y retrospectivo en pacientes atendidos en un servicio de nefrología pediátrica de tercer nivel. Se analizaron sexo, edad, estado nutricional, microorganismo aislado, talla renal, etiología de IVU, malformaciones renales. Las cicatrices renales se identificaron por gammagrafía; mientras que el daño renal incluyó disminución de la tasa de filtración glomerular (TFG), hipertensión arterial y proteinuria. Resultados: se incluyeron 48 pacientes, 43 mujeres, mediana de edad 48 meses; 89% con infección por Escherichia coli. El 39% presentó reflujo vesicoureteral, 25% obstrucción urinaria y 14.5% vejiga neurogénica. El 83% desarrolló cicatrización renal y el 62% daño renal, siendo en el 47% disminución de TFG. El único factor asociado a cicatrización y daño renales fue la detección de alteraciones en el ultrasonido renal. Conclusiones: la IVUr se asocia con alta frecuencia de nefropatía cicatricial y deterioro de la TFG. La identificación de alteraciones en ultrasonido renal es clave para la detección de pacientes con mayor riesgo de daño renal.
