        Background: Timely opioid access is essential in palliative care. Chile’s Law 21.375 and the Universal Palliative Care Program (CPU) expanded these services beyond oncological conditions, increasing territorial demand. Opioid availability in the Ñuble Health Service (SSÑ) network had not been characterized. Objective: To describe the territorial availability and distribution pattern of opioids for CPU patients in the SSÑ network, using conversion to oral morphine equivalents (OME). Methods: Descriptive and retrospective study (January–June 2025) based on institutional drug warehouse records (SIGBO) and REM P03. Distribution of transdermal buprenorphine, oral morphine, and injectable morphine was analyzed and expressed as total OME, OME per patient (semester), and daily OME per CPU patient. Results: A total of 355 CPU patients were registered, with 628,030 mg OME distributed. The regional average was 9.8 mg OME/day/ patient. Marked territorial heterogeneity was observed: some communes showed no opioid distribution despite having patients under control, while others exceeded 60 mg OME/day/ patient. Buprenorphine predominated over other formulations. Conclusions: This study establishes a regional baseline, identifies territorial gaps in opioid access, and provides evidence to guide training, prescribing, and logistics actions aimed at strengthening equity in the CPU program.
        Introducción: El acceso oportuno a opioides es esencial en cuidados paliativos. La Ley 21.375 y el Programa de Cuidados Paliativos Universales (CPU) extendieron estas prestaciones en Chile, incrementando la demanda territorial. La disponibilidad de opioides en la red del Servicio de Salud Ñuble (SSÑ) no estaba caracterizada. Objetivo: Describir la disponibilidad territorial y el patrón de distribución de opioides para pacientes CPU en la red SSÑ, mediante la conversión a miligramos de morfina oral equivalente (OME). Método: Estudio descriptivo y retrospectivo (enero–junio 2025) basado en registros de la Droguería institucional del SSÑ y REM P03. Se analizó la distribución de buprenorfina transdérmica, morfina oral y morfina inyectable, expresando la disponibilidad en OME total, OME/paciente (semestre) y OME/día/paciente. Resultados: Se registraron 355 pacientes CPU y se distribuyeron 628.030 mg OME. El promedio regional fue de 9,8 mg OME/día/paciente. Se observó marcada heterogeneidad territorial: algunas comunas no registraron egresos de opioides pese a tener pacientes bajo control, mientras otras superaron 60 mg OME/ día/paciente. Buprenorfina predominó sobre las demás presentaciones. Conclusiones: El estudio establece una línea base regional, identifica brechas en el acceso territorial a opioides y proporciona información para orientar acciones formativas, prescriptivas y logísticas que fortalezcan la equidad en el Programa CPU.
