One of the most striking statistics from the HRSA Health Workforce (March 2024) reveals that 195,292 registered nurses have left the workforce entirely. Background: This significant shift has accelerated a movement in healthcare -- empowering patients and caregivers with the education and tools needed to manage aspects of their own care. While self-care is not suitable for every patient, structured education and training programs have enabled appropriate individuals to take greater control of their health. This approach fosters autonomy, confidence, and engagement in their care journey. Discussion: At Wilmot Cancer Center in Rochester, NY, a multidisciplinary team has implemented a program to educate and train patients to disconnect their own chemotherapy. Of the approximately 358 patients receiving chemotherapy at home, 136 are now self-disconnecting. This initiative has not only enhanced patient independence but has also helped reduce the demand for home care nurses and alleviated volume pressures in infusion centres. Innovation: Enabling patients to self-disconnect represents a significant advancement in patient-centred care. By equipping them with the necessary skills and confidence, this approach fosters a greater sense of autonomy and control over their treatment.
