Sumario:INTRODUCTION - Heart transplantation is the gold standard treatment for endstage heart failure; however, prediction of postoperative outcomes remains a major clinical challenge. Owing to the limitations of conventional risk prediction models, increasing attention has been directed toward artificial intelligence-based approaches. PATIENTS AND METHODS - Our study was based on a retrospective database of 581 patients who underwent heart transplantation at the Heart and Vascular Center, Semmelweis University, between 2012 and 2024. Mortality was predicted using a random forest algorithm with nested crossvalidation, while predictor importance was assessed using SHAP analysis. RESULTS - The 30-day and 1-year mortality rates were 9.9% and 17.6%, respectively. Models based exclusively on preoperative data showed moderate predictive performance, whereas the incorporation of early postoperative parameters increased the area under the ROC curve to 0.98. SHAP analysis indicated that short-term mortality prediction was primarily driven by acute hemodynamic instability and early organ injury, whereas persistent organ dysfunction and markers of metabolic reserve were more important in longer-term prediction. CONCLUSIONS - Risk after heart transplantation changes dynamically over time, and the incorporation of early postoperative data may substantially improve mortality prediction. Our findings may provide a basis for the development of personalized clinical decision-support systems.
BEVEZETÉS - A szívtranszplantáció a végstádiumú szívelégtelenség gold standard kezelése, ugyanakkor a posztoperatív kimenetel előrejelzése továbbra is jelentős klinikai kihívást jelent. A hagyományos rizikóbecslő-modellek korlátai miatt egyre nagyobb figyelem irányul a mesterséges intelligencián alapuló módszerek alkalmazására. BETEGEK ÉS MÓDSZEREK - Vizsgálatunk a Semmelweis Egyetem Városmajori Szív-és Érgyógyászati Klinikáján 2012 és 2024 között szívtranszplantáción átesett 581 beteg retrospektív adatbázisán alapult. A mortalitás előrejelzésére random forest algoritmust alkalmaztunk nested cross-validation mellett, a prediktorok jelentőségét SHAPanalízissel értékeltük. EREDMÉNYEK - A 30 napos mortalitás 9,9%, az egyéves mortalitás 17,6% volt. A kizárólag preoperatív adatokra épülő modellek mérsékelt prediktív teljesítményt mutattak, míg a korai posztoperatív paraméterek bevonásával a ROC-görbe alatti terület 0,98-ra emelkedett. A SHAP-analízis alapján a rövid távú mortalitás predikciójában elsősorban az akut hemodinamikai instabilitás és a korai szervkárosodás, míg hosszabb távon a tartós szervdiszfunkció és a metabolikus rezerv mutatói bizonyultak meghatározónak. KÖVETKEZTETÉSEK - A szívtranszplantációt követő kockázat időben dinamikusan változik, és a korai posztoperatív adatok bevonása jelentősen javíthatja a mortalitás előrejelzését. Eredményeink alapot teremthetnek a személyre szabott klinikai döntéstámogatás fejlesztéséhez.
